This application form is for young people applying for affordable, fully furnished apartments in Derby. Please complete all sections as fully as possible to avoid delays in processing your application.

There are five sections to this form:

Section 2 – Your Current Living Situation

Section 3 – Employment and Income Information

Section 4 – Address History (past 3 years)

Section 5 – Declaration

You will not be able to save your form and come back to it, so please make sure you have all the relevant information ready. Before you start your Foundry Point housing application, please ensure you have:

Your National Insurance Number

Information about any benefits you receive (if applicable)

Your total monthly income from work (if applicable)

Address history for the past 3 years, including the dates you lived there, the amount of outstanding rent arrears if applicable, and the landlord’s name and contact details.
